A Home Safety Assessment thoroughly evaluates the client’s home environment using a checklist format. Thrive will use the checklist to work closely with the client to identify potential hazards and recommend practical solutions and modifications.
Upon completing the Home Safety Assessment, you will receive a detailed report outlining our findings, prioritized recommendations, and an implementation plan. We aim to empower you with the knowledge and tools to create a secure and comfortable living environment for you and your loved ones.

Functional Well-Being Assessments

A functional well-being assessment thoroughly evaluates your overall health and wellness. Thrive uses research-based, objective measurements to assess physical, cognitive, and emotional well-being. Thrive will develop an exercise and activity program that is meaningful and purposeful to you. We will create a baseline health report card and work with you to modify it as needed. Our goal is to empower you with the knowledge and tools to maintain or improve your functional well-being to continue to thrive while living safely and successfully in your home.


Personalized Senior Care Plan

Based on the findings and recommendations of the home safety and functional well-being assessments, Thrive will work directly with the client to create a meaningful and sustainable care plan regarding the home environment, exercise, and purposeful activity. Thrive will work with each client individually to determine the monthly frequency and duration. We aim to empower the client with ongoing guidance, tools, and strategies to control their overall well-being and live safely and successfully in their home.

Family and Caregiver Training

Based on the findings and recommendations of the home safety and functional well-being assessments, Thrive will provide hands-on training with family and caregivers to ensure the recommended tools and strategies are appropriately implemented. We aim to empower the client, family, and caregiver with tools, strategies, and ongoing guidance and support to live safely and successfully in your home.
